Care coordination for individuals affected by rare conditions survey

The Department of Health and Social Care has circulated a care coordination survey on CONCORD2 (COordiNated Care Of Rare Diseases 2). CONCORD2 is a research study that aims to examine the best approaches to care coordination for people affected by rare conditions, and develop recommendations about how care should be coordinated for these individuals.

The team has developed a survey to capture how the care of people affected by rare conditions is coordinated, and the costs and benefits of this to patients and families. It is hoped that the results of this survey will support improvements in how care is coordinated for people affected by rare conditions.
