Genetic Alliance’s ‘Coordinating Care: Learning from the experiences of people living with rare conditions’ has been published and is available to view.
They have worked with our members to demonstrate the value of care coordination and the impact of its absence. The benefits of care coordination are clear for people living with rare conditions and their carers, parents or relatives, for healthcare professionals, but also much more broadly for healthcare budgets, and providers of services outside of the healthcare environment.
